Your medical file

The GP medical station registers every medical request for help in a system. If you have been in contact with the GP medical station, we will send a message to your own GP after the service. Your GP will then know what the GP medical station has done for you. This report will be added to your medical file by your own GP. All telephone conversations are also recorded for quality purposes.

In the region, general practitioners and pharmacists join the LSP. The LSP stands for National Switch Point and is the care infrastructure of the Association of Care Providers for Healthcare Communication. With this computer system, general practitioners and assistants at the medical station can securely request a summary of your medical data.

Sharing your data

Your GP and pharmacy may only share your medical data with other healthcare providers if you give them prior permission to do so. Other healthcare providers may only request your data if this is necessary for your treatment.

Giving permission

You can give permission via the website www.volgjezorg.nl . You can fill in and send your details with DigiD. You can also print the form and give it to your healthcare provider. The GP medical station is also affiliated with the LSP because we think it is essential that medical information can also be viewed outside office hours.
